dc.description.abstractMogućnost CBCT imidžinga da preciznije odredi poziciju mete, anatomsku promenu pacijenta ili okolnog normalnog tkiva postao je neizostavan deo radioterapijskog tretmana, kada su u pitanju napredne tehnike planiranja, zbog velikog gradijenta doze. U kliničkoj praksi, CBCT imidžing se svakodnevno koristi kod sve većeg broja pacijenata, zbog čega je potrebna njegova sveobuhvatna procena kvaliteta i osobina, kao što su kvalitet slike i količina isporučene apsorbovane doze. Cilj ove doktorske disertacije je uspostavljanje adekvatne kontrole kvaliteta IGRT i optimizacije IGRT protokola za dva različita tipa akceleratora, koji su instalirani u celoj Srbiji, kako bi se dobio što kvalitetniji i bezbedniji radioterapijski tretman. Rad se sastoji iz dve celine. Prvi deo je teorijski uvod u kome se objašnjava potreba IGRT-a u radioterapiji, kao i potreba za optimizacijom IGRT-a u Srbiji; fizičke osnove rendgenskog zračenja, formiranja i karakteristika CBCT slike, kao i dozimetrija jonizujućeg zračenja. Takođe, u uvodnom delu je objašnjen menadžment IGRT gde će biti definisani pojmovi prijemnog testiranja, puštanja u rad i osiguranja kvaliteta kao i njihov značaj u radioterapiji. Drugi deo je eksperimentalni deo, koji sa sastoji od testova kontrole kvaliteta imidžing sistema dva različita tipa linearnih akceleratora, zatim od ispitivanja doze i kvaliteta slike, za regiju glave i vrata, torakalnu i pelvičnu regiju, nastala upotrebom kilovoltažnog CBCT imidžing sistema ioptimizacije IGRT prakse na Institutu za onkologiju Vojvodine primenjene na kliničkom primeru kraniospinalne iradijacije. Na samom kraju rada se nalaze zaključci u kome se nalaze preporuke primene IGRT, kao i smernice ka obezbeđivanju nacionalnog IGRT ptorokola.sr
dc.description.abstractThe ability of CBCT imaging to more precisely determine the position of the target, the anatomical change of the patient or the surrounding normal tissue has become an indispensable part of radiotherapy treatment, when it comes to advanced planning techniques, due to the large dose gradient. In clinical practice, CBCT imaging is used daily in an increasing number of patients, which is why a comprehensive evaluation of its quality and properties, such as image quality and the amount of delivered absorbed dose, is needed. The goal of this doctoral dissertation is to establish adequate quality control of IGRT and optimization of the IGRT protocol for two different types of accelerators, which are installed throughout Serbia, in order to obtain the highest quality and safest radiotherapy treatment possible. The work consists of two parts. The first part is a theoretical introduction explaining the need for IGRT in radiotherapy, as well as the need for optimization of IGRT in Serbia; physical basis of X-ray radiation, formation and characteristics of CBCT image, as well as dosimetry of ionizing radiation. Also, in the introduction, IGRT management is explained, where the concepts of acceptance testing, commissioning and quality assurance will be defined, as well as their importance in radiotherapy. The second part is the experimental part, which consists of the quality control tests of the imaging system of two different types of linear accelerators, then the dose and image quality tests, for the head and neck region, thoracic and pelvic region, created by the use of a kilovoltage CBCT imaging system and the optimization of IGRT practice at the Institute of Oncology of Vojvodina applied to the clinical example of craniospinal irradiation. At the very end of the paper, there are conclusions in which there are recommendations for the application of IGRT, as well as guidelines for the provision of a national IGRT protocol.en
