dc.description.abstractUvod: Podaci iz literature pokazuju da terapija antipsihoticima može da utiče na koštani metabolizam kod pacijenata sa shizofrenijom. Cilj: Naš cilj je bio da utvrdimo uticaj različitih antipsihotika na metabolizam kostiju, mineralnu koštanu gustinu i telesni sastav kod pacijenata sa shizofrenijom. Metode: Ispitivana populacija su pacijenti koji su više od 6 meseci na stabilnoj terapiji jednim antipsihotikom, po 20 u svakoj grupi (risperidon, klozapin i aripiprazol ) i 20 zdravih ispitanika bez postavljene dijagnoze mentalnog poremećaja. Obavljeno je merenje mineralne koštane gustine i telesnog sastava, određivanje markera koštanog metabolizma i biohemijskih analiza metaboličkih parametara, markera inflamacije, hormonalnog statusa i citokinskog profila. Rezultati: Statistički značajna razlika u vrednostima vitamina D i prolaktina je između pacijenta lečenih risperidonom prema aripiprazol grupi pacijenta i prema ispitanicima u kontrolnoj grupi. U grupi pacijenata lečenih risperidonom postoji statistički značajno negativna korelacija između koncentracije IL-17 i osteokalcina. U grupi pacijenata na terapiji klozapinom postoje statistički značajno pozitivne korelacije između citokina TNF-α i vrednosti BMD Neck; TGF-β sa BMD Total; IL-10 sa BMD Neck i IL17 sa BMD Neck, BMD Total i Z score Neck Total. U grupi pacijanata lečenih aripiprazolom postoje statistički značajno niže vrednosti BMD L1-L4, Z score L1-L4 u poređenju sa kontrolnom grupom. Zaključak: Pacijenti na terapiji antipsihoticima su u većem riziku za nastanak promena u mineralnoj koštanoj gustini. Mogući uzroci su hiperprolaktinemija, deficit vitamina D, promene u metaboličkom i hormonalnom statusu, kao i zbog izmena u citokinskom profilu.sr
dc.description.abstractIntroduction: Data from the literature show that antipsychotic therapy can affect bone metabolism in patients with schizophrenia. Aim: Our aim was to determine the effect of different antipsychotics on bone metabolism, bone mineral density and body composition in patients with schizophrenia. Methods: The studied population consists of patients who are on stable therapy with one antipsychotic for more than 6 months, 20 in each group (risperidone, clozapine and aripiprazole group) and healthy composition variants. Measurement of bone mineral density and body composition, determination of markers of bone metabolism and biochemical analysis of metabolic parameters, markers of inflammation, hormonal status and cytokine profile were performed. Results: There is a statistically significant difference in vitamin D and prolactin values between the patients treated with risperidone and the aripiprazole group of patients and the subjects in the control group. In the group of patients treated with risperidone, there is a statistically significantly negative correlation between the concentration of IL-17 and osteocalcin. In the group of patients on clozapine therapy, there are statistically significant positive correlations between the cytokine TNF-α and BMD Neck; TGF-β with BMD Total; IL-10 with BMD Neck and IL17 with BMD Neck, BMD Total and Z score Neck Total. In the group of patients treated with aripiprazole, there are statistically significantly lower values of BMD L1-L4, Z score L1-L4. Conclusion: Patients with antipsychotics are at greater risk at greater risk for changes in bone mineral density. Possible causes are hyperprolactinemia, vitamin D deficiency, changes in metabolic and hormonal status, as well as changes in the cytokine profile.en
