Uloga ove studije je da se na osnovu matematičkog modela i četiri parametra: telesne
težine, femoralnog offset-a, fizičke aktivnostii životnog stila, proceni trajnost femoralne
komponente endoproteze kuka.
Materijal i metode
U studiju je uključeno ukupno 18 bolesnika(12 žena i 6 muškaraca), uzrasta 35–55
godina kojima je 1990. godine, na IOHB „Banjica“, ugrađena totalna endoproteza kuka, i
na kojima je zatim, u istoj ustanovi, naknadno izvršena njena revizija. Kriterijumi za
uključivanje pacijenata u studiju bilisu: precizna veličina i pozicija femoralnog stema,
mladii aktivni pacijenti sa dobrim kvalitetom kosti, dok sukriterijumi za isključenje iz
studije bili: značajana displazija kuka, bolesnici sa sistemskim bolestima i bolesnici sa
značajnim preoperativnim invaliditetom.
Rezultati
Matematičkim modelom iizračunavanjem parametara na osnovu podataka za tri
bolesnika, uspeli smo da predvidimo trajnost femoralne komponente endoproteze kuka za
ostalih 15 bolesnika. Korist...ili smo Man-Vitni U test (Mann-Whitney U test) da bismo
uporedilirezultate dobijene matematičkim modelom i stvarne rezultate trajanja
endoproteze (p=0.713) koji su pokazaliizuzetnu preciznost matematičkog modela.
Statistička analiza urađenaje Statistica softverom (Statsoft, Dell). Prosečno trajanje
endoproteze kod bolesnika ženskog pola bilo je nešto duže (240.89 meseci) u odnosu na
pacijente muškog pola (199.5 meseci), ali razlika nije statistički značajna (MWU test,
p=0.07). Sa druge strane, telesna težina, dnevna aktivnost i femoralnioffset pokazalisu
značajnu statističku korelaciju (p< 0.05). Sobzirom na statističke rezultate, došlo se do
zaključka da ni godine starosti, kao ni pol, nemaju značajan uticaj na trajnost endoproteze
kuka...
The purpose of this work is to develop a new mathematical model that can accurately
predict the durability of a hip prosthesis due to the following four parameters: body
weight, femoral offset, duration and intensity of daily physical activities of a patient.
Methods
We retrospectively reviewed data of 18 patients (12 female and 6 male) who underwent
total hip arthroplasty in 1990 and who subsequently went on to revision surgery.The
average age of patients in this study group was 38.2 years (range 35–55 years). Inclusion
criteria for choosing patients were: accurate size and positioning of femoral stem, young
and active patients with good bone quality, while the exclusion criteria were: significant
dysplasia of the hip, patients with systemic diseases and patients with significant preoperative
functional disability.
Results
Based on mathematical model and the calculation of parameters by the data of three
patients, a formula was used to calculate the predicted durability o...f each person’s
prosthesis for 15 patients to assess the compliance between predicted and actual
results.The Mann-Whitney U test of ranks was performed to compare the distribution of
calculated values and recorded values of the prosthesis duration, resulting in acceptance
of the hypothesis of their equality (p = 0.713).The average duration of prostheses in
female patients was slightly higher (240.89 months) than in male patients (199.5 months),
but this difference did not perform as statistically significant (MWU test, p = 0.119).On
the other hand, body mass (Q), daily exercise activity (T) and femoral offset (h) have all
proven to have a statistically significant correlation (p<0.05). Therefore, we can conclude
that neither age nor gender have a significant impact on the expected duration of
prostheses...
