dc.description.abstractUvod. Nagluvost i gluvoća predstavljaju najčešće senzorno oštećenje u ljudi, sa značajnim socijalnim i psihološkim implikacijama. Od osam faktora rizika za njihov nastanak dva su infekcije: rubella, Citomegalovirus i bakterijski meninigitis. Zastupljenost faktora rizika je različita u različitim regionioma i periodima posmatranja. Kod svakog deteta sa senzorineuralnim oštećenjem sluha neophodno je sprovesti ispitivanje kojim bi se odredio etiološki faktor jer je važno za donošenje odluke o lečenju i habilitaciji, za procenu prognoze, za kliničko praćenje kao i za dalje planiranje porodice. Ciljevi istraživanja. Ciljevi su nam bili da sagledamo učešće infektivnih bolesti u nastanku trajnog oštećenja sluha u dečjem uzrastu u populaciji bolesnika tercijarne pedijatrijske ustanove u Srbiji u sadašnjem vremenu; da ispitamo audiološke i elektrofiziološke karakteristike odgovora evociranih potencijala moždanog stabla u navedenim slučajevima. Materijal i metode. Istraživanje je dizajnirano kao studija slučajeva i kontrola. Istraživanje je sprovedeno u Odseku za audiologiju i neurootologiju Službe za otorinolaringologiju Instituta za zdravstvenu zaštitu majke i deteta Srbije „Dr Vukan Čupić“ tokom perioda od 01. 01. 2000. do 31. 12. 2014. retrospektivno. Ispitana su 532 pacijenta sa oštećenjem sluha u populaciji od 14000 pacijenata. Rezultati. Potvrdili smo radne hipoteze da su infektivni agensi i danas značajni faktori rizika za nastanak trajnog oštećenja sluha kod dece kao i da ono ima određene audiološke i elektrofiziološke karakteristike koje ga razlikuju od oštećenja druge etiologije. Meningitis, rubella i CMV, zajedno, kao infektivni agensi, učestvuju sa 7,33% u nastanku različitih oblika oštećenja sluha u našoj grupi ispitanika. Zaključci. U studiji slučajeva i kontrola ustanovili smo preko izračunavanja odnosa šansi da je mnogo veća mogućnost za nastajanje senzorineuralnog oštećenja sluha u grupi izložene dece nego u kontrolnoj. Potvrdili smo hipotezu o odstupanju vrednosti određenih elektrofizioloških parametara kod pomenute dece od vrednosti kod dece iz drugih etioloških kategorija uključujući i one urednog sluha.sr
dc.description.abstractIntroduction. Hearing loss and deafness are the most common sensory impairments with significant social and psychological implications. Out of eight risk factors for their development two are infections: rubella, cytomegalovirus and bacterial meningitis. Prevalence of risk factors is different in different observation periods and regions. For each child with a hearing loss it is necessary to determine the etiologic factor in order to make decisions on treatment and habilitation, prognosis assessment, clinical monitoring and further family planning. Aim of study. Our aim was to look into the participation of infectious diseases in the development of permanent hearing impairment in paediatric patient population of Serbian tertiary institution at present and to examine the characteristics of audiological and electrophysiological responses of evoked brainstem potentials in these cases. Method.We performed case and control study in the Institute for Mother and Child Health Care of Serbia “Dr Vukan Čupić” during the period from 01. 01. 2000. until 31. 12. 2014. retrospectivly. We have examined 532 patients with hearing impairment in a population of 14000 patients. Results. In the study of cases and controls we calculated that there was a much greater likelihood for the development of sensorineural hearing impairment in the group of children exposed to the infection than in the control. Meningitis, rubella and CMV, as infectious agents, together accounted for 7.33% in the emergence of various forms of hearing impairment in our group of patients. Conclusions.We have confirmed the working hypothesis that the infectious agents still constitute significant risk factors for the development of permanent hearing loss in children as well as the hypothesis of their deviating values of certain electrophysiological parameters compared to both those in other etiologic categories and those with normal hearing.en
