Određivanje skora kalcijuma koronarnih arterija kao skrining rane detekcije ishemijske bolesti srca kod asimptomatskih ispitanika sa dijabetesom

Abstract
Uvod: Ishemijska bolest srca je progresivnija kod pacijenata sa dijabetesom . OdreĎivanje skora kalcijuma koronarnih arterija je neinvazivna metoda otkrivanja kalcifikacija koronarnih arterija, i dobar pokazatelj nastanka nepovoljnog kardiovaskularnog dogaĎaja. Metodologija: U našoj studiji analizirane su dve grupe asimptomatskih ispitanika, sa i bez dijabetesa . Svim ispitanicima izračunat je skor kalcijuma koronarnih arterija-AS, odreĎivanje koronarne rezerve protoka-CFR, ehokardiografski pregled, odreĎen je intima medija kompleks na nivou zajedničke karotidne arterije-IMK, stresehokardiografski test, kao i biohemijski i metabolički parametri. Svi ispitanici su praćeni minimum godinu dana u odnosu na pojavu glavnih neţeljenih kardiovaskularnih dogaĎaja. Rezultati: Dokazano je da su značajno viši ukupni skor kalcijuma koronarnih arterija,kao i pojadinačni skorovi kod ispitanika sa dijabetesom. Značajno je veća učestalost pozitivnog stres– eho testa, p<0,0001, češće su povišene vrednos...ti IMK , p<0,0001 , veća je zastupljenost ispitanika sa CFRispod 2, niţa prosečna vrednost CFR, p<0,001.Veća je učestalost ispitanika sa sniţenom EF LK, p<0,002 , sa poremećajem kinetike leve komore u miru medju ispitanicima sa dijabetesom. Neţeljeni klinički dogadjaj imalo je 18,8% ispitanika sa dijabetesom, 5,1% ispitanika bez dijabetesa, p<0,003. Neţeljeni klinički dogadjaj tokom praćenja imalo je 13 ispitanikasa dijabetesom i skorom kalcijuma preko 400 Agatstonovih jedinica, a samo dva ispitanika bez dijabetesa.Odnos šanse da ispitanici sa dijabetesom imaju neţeljeni dogadjaj je 4,36puta veća . Učestalost neţeljenih dogadjaja je viša medju pacijentima sa dijabetesom sa CFRLAD do 2 i sa višim skorom kalcijuma. Na više vrednosti skora kalcijuma (preko 200 ) kod ispitanika sa dijabetesom kao najjači prediktori izdvojili su se viši stepen 10 godišnjeg rizika (od 10% i više) za KVB (OR=5,21) , pojava neţeljenog kliničkog dogadjaja (OR=22,87), povišena IMK (OR=10,38) , kao i pozitivan stres eho test (OR=5,67). Zaključak: Prisustvo dijabetesa utiče na vrednosti skora kalcijuma, jedan od najznačajnijih prediktora za pojavu neţeljenih kardiovaskularnih dogadjaja u praćenju asimptomatskih ispitanika...

Objective: Ischemic heart disease is more progressive in patients with diabetes. Determination of coronary artery calcium score is a non-invasive method for detection of coronary artery calcification, and also a good indicator of the occurrence of adverse cardiovascular events. Methods: In our study we analysed two groups of asymptomatic subjects with and without diabetes. All participants undervent coronary calcium measurement according to Agatston method, coronary flow velocity reserve-CFVR, echo, stress echo test and measurement of intimamedia thickness (complex)-IMK. We analysed biochemical and metabolic parameters. All participants were followed minimum one year due to occurrence of mayor adverse cardiovascular events (MACCE). Results: It has been demonstrated that patients with diabetes had a significantly higher total coronary calcium score , as well as individual scores, and those significantly correlated with higher frequency of positive stress -echo test , p < 0.0001, had mor...e often higher values of IMK , p < 0.0001 , the CFR values below 2 , a lower average value of the CFR , p <0.001 . There was higher frequency of diabetic patients with a reduced LV EF, p <0.002, and rest left ventricle wall motion abnormalities. Adverse clinical events had 18.8 % of the subjects with diabetes and 5.1 % of subjects without diabetes, p<0.003. Adverse clinical events during minimum 12 months follow-up had 15 subjects with diabetes and calcium score 4, and only one participant without diabetes. The odds ratio that the subjects with diabetes have a significant adverse event was 4.36 times higher. The incidence of adverse events was higher in patients with diabetes with CFR LAD to 2 and with higher calcium score. The strongest predictors of higher calcium score values ( over 200 ) were : a higher level of 10 year risk ( 10 % or more) for CVD ( OR = 5.21 ) , as well as the occurrence of adverse clinical events ( OR = 22.87) , elevated IMK ( OR = 10.38 ) ,and positive stress echo test ( OR = 5.67 ) . Conclusion: The presence of diabetes affects the calcium score values, and calcium score is the one of the most important predictors for the major adverse cardiovascular events.
