dc.description.abstractLečenje opekotinskih rana predstavlja veliki klinički izazov. Izbor idealnog topikalnog sredstva koje bi trebalo da poseduje sve pozitivne karakteristike predstavlja i dalje problem pred kojim se suočava naučna javnost. Ova studija je sprovedena kao prospektivna, eksperimentalna studija na Naučnom Institutu za Stočarstvo-Zemun. Obuhvatila je ukupno 72 opečene površine na 9 životinja, koje su podeljene u tri eksperimentalne grupe (Brufen mast, Manuka med, ekstradevičansko maslinovo ulje) i jednu kontrolnu grupu (kombinacija antibiotskih masti). U studiji se na višestrukim nivoima uticalo na: dubinu opekotinske rane, antimikrobnu, antioksidativnu i antiinflamatornu aktivnost i potom su analizirani postignuti rezultati zarastanja i modulacije ožiljka. Za animalni model je izabrana svinja plemenite rase Landras zbog svojih histoloških i fizioliških sličnosti sa ljudskom kožom. Rezultati su potvrđeni patohistološkim, histohemijskim (Picrosirius red) i imunohistohemijskim (Iba1, Ki67 i pancitokeratin) analizama. Najbrže zarastanje je zabeleženo kod grupe u kojoj je korišćen manuka med kao topikalno sredstvo, što se može tumačiti višestrukim pozitivnim delovanjem meda na okruženje rane. Kod grupe u kojoj je korišćena Ibuprofen mast je zabeleženo statistički značajno inicijalno smanjenje dubine i ubrzavanje reepitelizacije opekotinske rane zahvaljujući pozitivnom delovanju inhibitora cikloksigenaze na zonu staze i hiperperfuziju. Grupa kod koje je korišćeno ekstradevičansko maslinovo ulje pokazala je statistički značajno ubrzanu proliferativnu fazu zarastanja i bržu reepitelizaciju od kontrolne grupe. Najbolji estetski rezultat i pozitivan uticaj na modulaciju ožiljka je imala grupa kod koje je korišćen manuka med gde je utvrđen najtanji sloj organizovanog granulacionog tkiva i epidermisa u odnosu na ostale ispitivane grupe. U ovoj studiji je potvrđeno da se infracrvena kamera (Flir one) može koristiti u svakodnevnom kliničkom radu, te da se sa velikom sigurnošću može odrediti dubina opekotinske rane. Pravci daljeg istraživanja koji bi omogućili pronalaženje kombinovanog topikalnog sredstva sa idealnim osobinama bi mogli značajno doprineti lečenju opekotina granične dubine, ubrzali zarastanje i smanjili broj postopekotinskih sekvela.sr
dc.description.abstractBurn wound treatment is a significant clinical challenge. The scientific community is currently struggling with how to choose an optimal topical drug that should have every beneficial quality. This investigation was carried out at the Scientific Institute for Animal Husbandry-Zemun as a prospective, experimental study. On nine animals, 72 burned regions were coated in total. Three experimental groups—using Brufen ointment, Manuka honey, and extra virgin olive oil—and one control group—using an antibiotic ointment combination—were used. The depth of the burn wound, antibacterial, antioxidant, and anti-inflammatory activity was affected in the study on various levels, and the outcomes of healing and regulation of the scar were then examined. Because of the noble Landras pig's histological and physiological resemblance to human skin, it was chosen as the animal model. Patho histological, histo-emic (Picrosirius red), and immuno-histo-emic (Iba1, Ki67, and pancytokeratin) studies all supported the findings. The group who applied manuka honey topically experienced the quickest healing, which can be attributed to the honey's several advantageous effects on the environment around the lesion. The cyclooxygenase inhibitor's beneficial effects on the stasis zone and hyperperfusion led to an initial decrease in depth and an acceleration of re-epithelialization of the burn wound that were statistically significant when measured with ibuprofen ointment. Extra virgin olive oil was used in the group, and this group outperformed the control group in terms of rapid proliferative healing and quicker reepithelialization. In comparison to the other groups under study, the group that utilized manuka honey had the best cosmetic outcome and a favorable effect on scar modification. This group also had the smallest layer of organized granulation tissue and epidermis.The infrared camera (Flir one) can be used in routine clinical work, and it was proven in this study that it can accurately measure the depth of burn wounds. The results of additional study that would enable the discovery of a combination topical drug with perfect qualities could greatly aid in treating borderline depth burns, hasten healing, and lessen the incidence of burn sequelae.en
