Uticaj suplementacije omega-3 i omega-6 masnim kiselinama na masnokiselinski profil plazme, oksidativni stres i inflamatorni odgovor kod pacijentkinja na adjuvantnoj terapiji karcinoma dojke

Effect of omega-3 and omega-6 fatty acid supplementation on plasma fatty acid profile, oxidative stress, and inflammatory response in breast cancer patients during adjuvant therapy

Abstract
Uvod: Karcinom dojke (KD) je drugi vodeći uzrok smrti kod žena. Osim tumora, hemoterapija i dugotrajna endokrina terapija inhibitorima aromataze (IA) mogu nepovoljno da utiču na metabolizam lipida, inflamaciju i nutritivni status pacijenata. Cilj: Proceniti efikasnost ribljeg ulja i ulja žutog noćurka, izvorima polinezasićenih masnih kiselina (PUFA), na nutritivni i lipidni status, nivo citokina i oksidativni stres kod pacijentkinja sa KD na hemoterapiji i terapiji IA. Metode: U studiju je uključeno 80 postmenopauznih žena, starosti 45-70 godina, sa estrogen i/ili progesteron receptor pozitivnim karcinomom dojke, koje su na hemoterapiji ili terapiji IA najmanje 2 godine. Studiju je završilo 60 žena. Grupe su bile podeljene u dve podgrupe, od kojih je interventna uzimala dve gel kapsule ribljeg ulja sa ukupno 1 g n-3PUFA, i dve gel kapsule ulja žutog noćurka, sa 234 mg gama-linolenske kiseline tokom 12 nedelja uz redovnu terapiju. Druga grupa je uzimala placebo kapsule. Rezultati: Rezul...tati studije su pokazali značajno povećanje svih n-3PUFA i smanjen nivo n-6PUFA i odnos n-6/n3PUFA u obe interventne grupe. Suplementacija nije uticala na lipidni profil i nutritivni status pacijentkinja. Nivo interleukina IL6 u interventnoj grupi kod žena na hemoterapiji je smanjen, kao i nivo IL-10 kod žena koje su primale IA. U obe interventne grupe povećan je stepen lipidne peroksidacije. Suplementacija PUFA značajno je smanjila nivo H2O2 i aktivnost katalaze kod žena na hemoterapiji, dok je kod žena na terapiji IA povećala parametre oksidativnog stresa. Zaključak: Suplementacija ribljim uljem i uljem žutog noćurka ima blagotvorne efekte na status PUFA kod pacijenata sa KD na hemoterapiji i terapiji IA, ali uticaj ovih suplemenata na inflamaciju i oksidativni stres zavise od primarne terapije.

Introduction: Breast cancer (BC) is the second leading cause of cancer-related death in women. Besides the tumor itself, chemotherapy (CT) and prolonged endocrine therapy with aromatase inhibitors (AI), may influence lipid metabolism, nutritional status, and inflammation. Objective: To evaluate the efficacy of fish oil (FO) and evening primrose oil (EPO), rich in polyunsaturated fatty acids (PUFA), on serum lipids, nutritional status, inflammation and oxidative stress in BC patients receiving CT or AI therapy. Methods: In this study we included 80 postmenopausal women, aged 45- 70, with BC, receiving CT or AI therapy for at least 2 years. Sixty women completed the study. Both groups were divided into 2 subgroups: the intervention group, taking 2 gel capsules of FO containing a total of 1 g n-3 PUFA and two gel capsules of EPO, containing 234 mg of gamma-linolenic acid during 12 weeks with regular therapy, and the placebo group. Results: Results of this study showed significantly increa...sed levels of all n-3PUFA and decreased n-6PUFA in the intervention groups. No effects on lipids and nutritional status were found. There was a significant decrease in the level of Interleukin IL-6 in the intervention group in women on CT, while the level of IL-10 decreased in women receiving AI therapy. In both interventional groups level of lipid peroxidation increased. The supplementation decreased the level of H2O2 and catalase activity in women receiving CT, while in women with AI therapy markers of oxidative stress increased. Conclusion: The supplementation with FO and EPO has beneficial effects on PUFA status in patients with BC on CT and AI therapy, but the impact of these supplements on inflammation and oxidative stress parameters largely depends on the type of primary therapy.
