The influence of piezoelectric effect on a cellular and molecular mechanism of oralsurgical wound healing in patients with diabetes mellitus type 1 and 2

dc.descriptionTrauma mekog i koštanog tkiva u toku hirurške ekstrakcije impaktiranih umnjaka u donjoj vilici se klinički manifestuje pojavom bola, otoka i trizmusa, a koje nastaju kao posledica inflamatornih procesa u tkivima. Težina postoperativnog oporavka pacijenata zavisi od karakteristika koštanog tkiva oko impaktiranog zuba, kao i od stepena hirurške traume, koja se javlja u toku uklanjanja koštanog tkiva da bi se pristupilo vađenju zuba. Pijezohirurško sečenje kosti, kao minimalno invazivan hirurški pristup, doprinosi smanjenju hirurške traume alveolarnog koštanog tkiva i okolne mukoze i na taj način doprinosi prevazilaženju ograničenja konvencionalnih pristupa u ekstrakcijama donjih impaktiranih umnjaka. Štaviše, rezultati histoloških i histomorfometrijskih analiza koštanog tkiva na animalnom modelu ukazali su da su pijezohirurške osteotomije, za razliku od konvencionalnih, praćene intenzivnijim stvaranjem koštanog tkiva, kao i smanjenom broju inflamatornih ćelija. Trauma intraoralnog mekog i koštanog tkiva u toku hirurške ekstrakcije donjih impaktiranih umnjaka, kao i u toku procedura komplikovanih ekstrakcija višekorenih zuba je najvećim delom uslovljena intenzitetom povređivanja tkiva primenjenom hirurškom tehnikom, kao i posledičnim inflamatornim odgovorom organizma. Preterano oslobađanje pro-inflamatornih i pro-angiogenih molekula, kao što je interleukin 1 (IL- 1), faktor rasta vaskularnog endotela (VEGF) i slobodnih kiseoničnih radikala (ROS) utiče na intenzitet pojave bola, otoka i ograničenog otvaranja usta nakon hirurške ekstrakcije umnjaka u donjoj vilici. Pacijenti oboleli od dijabetes melitusa (DM) imaju povećan rizik za pojavu otežanog zarastanja koštanog i mekog tkiva, kao i mogućnost za nastankom postoperativnih infekcija rane. Mehanizmi koji učestvuju u patofiziologiji poremećaja mekotkivnog i koštanog zarastanja u stanjima DM zasnivaju sa na međusobnoj povezanosti hiperglikemije, pro-inflamatorne ćelijske signalizacije i oksidativnog stresa. Molekule koje se od skoro smatraju bitnim faktorom u regulisanju inflamatornog odgovora i oksidativnog stresa u DM pripadaju familiji mikroRNK molekula (miRs). Štaviše, miR-21, jedna od najviše izučavanih molekula miR-a, predstavlja važnu kariku u regulaciji inflamatornog odgovora organizma kao posledicu hirurške traume tkiva. Konačno, miR-21 je uključena u kontroli bioloških funkcija epigenetskom modulacijom svojih target gena, odnosno kontrolom njihove transkripcije i posledične sinteze faktora rasta, kao što su matriksna-metaloproteinaza- 9 (MMP-9), fosfataza i tenzin homolog (PTEN) i aktivnost enzima superoksiddizmutaze (SOD)...sr
dc.descriptionPostoperative pain, trismus, and swelling reflect inflammatory events following removal of impacted mandibular third molar and are mostly related to surgical injury. The occurrence of postoperative discomfort is associated with bone properties, as well as with degree and mode of bone removal. Piezosurgical mode of bone cutting is a minimally invasive surgical approach that may reduce osteotomyassociated injury to alveolar bone and surrounding soft tissue, and helps overcome limitations of conventional third molar surgical treatment. Furthermore, histological and histomorphometric evidence in animal models demonstrated increased bone formation, as well as lower number of inflammatory cells in sites treated by piezoelectric devices compared with sites where conventional bone-cutting was used. Surgical trauma to the soft and hard tissue during removal of impacted mandibular third molars, as well as complex teeth exodontia is mostly influenced by the extent of tissue injury and consequential inflammation. The substantial production of proinflammatory and pro-angiogenic molecules, such as Interleukin 1 (IL-1), Vascular Endothelial Growth Factor (VEGF) and Reactive Oxygen Species (ROS) results in postoperative outcomes such as: pain, swelling, and limited mouth opening. Patient with diabetes mellitus have an increased risk for impaired bone and soft tissue healing and they can experience higher occurence of postoperative infection. Mechanism underlying diabetes-induced impairment of bone and soft tissue healing were attributed to the muttually interrelated processes of hyperglycemia, proinflammatrory signaling and oxidative stress. Molecules that have recently emerged as critical factors in regulation of inflammation and oxidative stress in DM are the microRNAs (miRs). Moreover, one of the mostly studied miRs is miR-21 and has been recognized as molecular regulators of inflammation following tissue injury.
