dc.description.abstractKorišćenje preventivnih zdravstvenih usluga prepoznato je kao dugoročno zdravstveno ulaganje zbog svog potencijala da pomogne pojedincima da brinu o svom zdravlju. Ova studija imala je za cilj da istraži dostupnost i pružanje preventivnih zdravstvenih usluga u primarnoj zdravstvenoj zaštiti (PZZ) u oblastima rada opšte medicine, pedijatrije i ginekologije, kao i da analizira uticaj sociodemografskih i zdravstvenih faktora na pružanje preventivnih zdravstvenih usluga. Metod: Ova studija preseka koristila je podatake Republičkog fonda za zdravstveno osiguranje i Republičkog zavoda za statistiku Srbije za 2015. i 2018. godinu. U analizu je uključeno 149 opština sa nezavisnim jedinicama PZZ. Za ispitivanje povezanosti između pružanja preventivnih usluga i sociodemografskih i zdravstvenih karakteristika stanovništva korišćene su bivarijantne i multivarijantne regresione analize. Rezultati: U modelu višestruke regresije za domen pedijatrije, veći broj pruženih preventivnih zdravstvenih usluga na 1.000 dece, imale su opštine gde su deca imala višu stopu završetka osnovne škole (p=0.024) u 2015 i (p=0.023) 2018. god. Veći izdaci za zdravstvenu zaštitu (p=0.017) u 2015. god, veće očekivano trajanje života na rođenju (p=0.041) u 2015. god. i deca iz urbanih sredina u 2018. god. (p=0.020) takođe su promenljive koje su bile pozitivno povezane sa brojem pruženih preventivnih zdravstvenih usluga kod izabranog lekara. Što se tiče oblasti opšte medicine, višestruka regresiona analiza pokazala je da odraslo stanovništvo (19+) sa većim izdvajanjima za zdravstvenu zaštitu po glavi stanovnika (p=0.006) u 2015. god. i većom stopom završetka osnovne škole (p=0.004) u 2018. god., kao i većom prosečnom zaradom (USD) (p=0.018) u 2018. god. u većoj meri koristi preventivne usluge na 1.000 odraslih. Što je bila veća smrtnost od hroničnih nezaraznih bolesti (p=0.003) u 2015.god. i (p=0.012) u 2018. god. to je više preventivnih usluga pružio izabrani lekar opšte medicine. U ginekološkoj oblasti jedina promenljiva koja je bila značajno povezana sa brojem preventivnih zdravstvenih usluga na 1.000 žena u višestrukoj linearnoj regresiji bila je stopa plodnosti (p=0.003) u 2015. god. Ženska populacija (15+) sa većom stopom završetka osnovne škole (p=0.011) i koja živi u urbanim sredinama (p=0.046) u 2018. god. više je koristila preventivne usluge kod izabranog ginekologa u PZZ. Promenljive koje su pokazale pozitivnu korelaciju (r) sa upotrebom preventivnih usluga i značajnu razliku (p<0.001) između dva ispitivana perioda (2015-2018. god.) su: demografske - urbana populacija (r=0.995), udeo populacije 65+ godina (r=0.993); socioekonomske - stopa nezaposlenosti (r=0.951), proseča zarada zaposlenih (r= 0.826), rashodi za zdravstvenu zaštitu (r=0.812) i zdravstvene karakteristike populacije - ukupni mortalitet od HNB (r=0.899) i mortalitet od kardiovaskularnih bolesti (r=0.894). Od indikatora dostupnosti izabranih lekara i performansi usluga u PZZ, pozitivnu korelaciju (r) sa brojem ostvarenih preventivnih usluga (2015-2018. god.) pokazali su, u opštoj medicini: broj lekara (p<0.001; r=0.949), broj lekara na 10.000 ciljne populacije (p<0.001); r=0.789) i ukupan broj poseta po izabranom lekaru (p<0.009; r=595), dok su u pedijatriji: broj lekara (p<0.044; r=0.944) i u ginekologiji: broj lekara na 10.000 ciljne populacije (p<0.028; r=0.698). Zaključak: Ova studija identifikovala je faktore koji utiču na pružanje preventivnih zdravstvenih usluga u PZZ. Najvažniji prediktori korišćenja preventivnih zdravstvenih usluga su stopa završetka osnovne škole, procenat urbane populacije, prosečna zarada zaposlenih (USD), ukupna stopa smrtnosti od HNB i broj izabranih lekara u PZZ.sr
dc.description.abstractThe use of preventive health services has been recognized as a long-term health investment because of its potential to help individuals take care of their health. This study aimed to investigate the availability and provision of preventive health services in primary health care (PHC) in the fields of general medicine, pediatrics and gynecology, as well as to analyze the impact of sociodemographic and health factors on the provision of preventive health services. Method: This cross-sectional study used data from the Republic Health Insurance Fund and the Republic Bureau of Statistics of Serbia for 2015 and 2018. The analysis included 149 municipalities with independent PHC units. Bivariate and multivariate regression analyzes were used to examine the relationship between the provision of preventive services and the sociodemographic and health characteristics of the population. Results: In the multiple regression model for the domain of pediatrics, a larger number of provided preventive health services per 1,000 children were in municipalities where children had a higher primary school completion rate (p = 0.024) in 2015 and (p = 0.023) 2018. Higher health care expenditures (p = 0.017) in 2015, higher life expectancy at birth (p = 0.041) in 2015. and children from urban areas in 2018. (p = 0.020) are also variables that were positively related to the number of preventive health services provided by the chosen physician. Regarding the field of general medicine, multiple regression analysis showed that the adult population (19+) with higher spending for health care per capita (p = 0.006) in 2015. and a higher primary school completion rate (p = 0.004) in 2018. And higher average earnings (USD) (p = 0.018) in 2018. it used more preventive services per 1,000 adults. The higher the mortality from chronic non-communicable diseases (p = 0.003) in 2015. and (p = 0.012) in 2018. that is, more preventive services were provided by a chosen general practitioner. In the gynecological field, the only variable that was significantly related to the number of preventive health services per 1,000 women in multiple linear regression was the fertility rate (p = 0.003) in 2015. Female population (15+) with a higher completion rate of primary school (p = 0.011) and living in urban areas (p = 0.046) in 2018. used preventive services a chosen gynecologist in PHC. Variables that showed a positive correlation (r) with the use of preventive services and a significant difference (p <0.001) between the two examined periods (2015-2018) are: demographic - urban population (r = 0.995), share of population 65+ years (r = 0.993); socio-economic - unemployment rate (r = 0.951), average salary of employees (r = 0.826), health care expenditures (r = 0.812) and health characteristics of the population - total mortality from the CND (r = 0.899) and mortality from cardiovascular diseases (r = 0.894). Regarding indicators of availability of selected doctors and performance of services in PHC, a positive correlation (r) with the number of realized preventive services (2015-2018) was noted, in general medicine: number of doctors (p <0.001; r = 0.949), number of doctors per 10,000 target population (p <0.001); r = 0.789) and the total number of visits per selected physician (p <0.009; r = 595), while in pediatrics: number of physicians (p <0.044; r = 0.944) and in gynecology: number of physicians per 10,000 target populations (p <0.028 ; r = 0.698). Conclusion: This study identified factors influencing the provision of preventive health services in PHC. The most important predictors of the use of preventive health services are the primary school completion rate, the percentage of the urban population, the average salary of employees (USD), the total mortality rate from the CND and the number of selected doctors in PHC.en
