Uvod: Starenje je univerzalna pojava i kao takva pobuđuje opšti društveni interes.
Sa aspekta neuronauka, starenje je predmet interesovanja zbog neurodegenerativnih
promena koje mogu dovesti do promena u kognitivnom, psihološkom i fizičkom
funkcionisanju. Razumevanje uzročno-posledičnih veza između deteriorirajućih
mentalnih i fizičkih procesa, skopčanih sa starenjem, koji potencijalno mogu voditi
ka gubljenju rezervnih kapaciteta odnosno nastanku fragilnosti, je od kritičnog
značaja, pošto može pomoći u unapređenju preventivnih i terapeutskih strategija.
Glavni cilj ove disertacije bio je da istraži međusobnu povezanost kognitivnog,
emocionalnog, fizičkog funkcionisanja i navika u ishrani kod osoba starijeg
životnog doba kao i povezanost ovih kategorija sa fizičkom aktivnošću i na taj
način, bliže utvrdi karakteristike fragilnosti-staračke nemoći.
Metode: Korelaciona, transverzalna studija u kojoj je učestvovalo 140 ispitanika
starosne dobi od 65 i više godina, o...ba pola. Od ukupnog broja ispitanika, formirane
su dve grupe prema kriterijumu bavljenja fizičkom aktivnošću. Kod ispitanika je
utvrđivan indeks telesne mase (BMI). U istraživanju su korišćeni testovi i skale za
ispitivanje kognitivnih funkcija: Montrealska skala kognicije (MoCA), Test „Idine idi (Go no go), Testovi kategorijalne i fonemske fluentnosti. Fizičkomotoričko funkcionisanje ispitano je upotrebom: Dinamometrijske metode stiska
šake, testom Brzine hoda, Bergovom skalom balansa (BBS), Vremenskim „ustani i
kreni“ test (TUG), Senior fitnes testom (SFT). Funkcija spavanja ispitana je
Pitsburškim indeksom kvaliteta sna (PSQI), a prisustvo simptoma depresije
Gerijatrijskom skalom depresije (GDS) - kratka forma. U istraživanju su
upotrebljeni i: Upitnik za nutritivni skrining „DETERMINE“ kao i upitnici za
identifikovanje fragilnosti i onesposobljenosti „FIND“ i Prizma 7.
Rezultati: Fizički aktivni ispitanici imaju u proseku nešto niži BMI, i ova
razlika je statistički značajna. Interkorelacije kognitivnih dimenzija testa MoCA
(egzekutivne funkcije, jezik, vizuoprostorne veštine, pamćenje, pažnja,
koncentracija, radna memorija i orijentacija) su pozitivne, umerene do visoke, i
najveći broj korelacija je statistički značajan. Dakle, viši skorovi na jednoj
dimenziji znače više skorove na preostalim dimenzijama. Podgrupa osoba koje se
rekreativno bavi fizičkom aktivnošću pokazuje, statistički značajno bolje rezultate
na Montrealskoj skali kognicije, na testu „Idi-ne-idi“, kao i na Testovima
kategorijalne i fonemske fluentnosti. Ispitanici koji su fizički aktivni postižu
značajno niže skorove od ispitanika koji nisu fizički aktivni na Gerijatrijskoj
skali depresije odnosno, simptomi depresije su više izraženi kod fizički
neaktivnih ispitanika. Na Pitsburškom indeksu kvaliteta sna, grupa fizički
aktivnih ima statistički značajno niže skorove od grupe fizički neaktivnih, što
znači da sveukupno ima bolji kvalitet sna. U pogledu fizičko – motoričkih testova,
fizički aktivni ispitanici imaju bolje postignuće (izvršavaju više zadatih radnji
za određeno vreme, odnosno potrebno im je manje vremena za zadatu radnju) na svim
motoričkim testovima i razlika u odnosu na postignuće fizički neaktivnih je
statistički značajna i prema parametrijskoj i neparametrijskoj analizi. Od
dinamometrijskih parametara stiska šake, maksimalna sila stiska šake (Fmax) obe
šake je povezana sa brzinom hoda – ispitanici kojima je trebalo manje vremena da
pređu zadatu distancu demonstrirali su veću maksimalnu mišićnu silu na obe šake.
Određene kognitivne kategorije, povezane su sa većom mišićnom snagom stiska šake.
Postoje korelacije između kognitivnih funkcija i simptoma depresije, a sa
depresivnošću najsnažnije je povezan skor na MoCA. Ispitanici koji imaju bolji
motorički status (kraće vreme, više ponavljanja, itd) imaju i niže skorove na
upitniku o kvalitetu sna, koji ukazuju na bolji san. Ispitanici koji imaju više
skorove na skali depresivnosti imaju nešto lošiji motorički status. Utvrđene su
korelacije između FIND i Prizma 7 skorova i: maksimalnog stiska šake leve i desne
ruke; TUG, brzine hoda, SFT-a i BBS skorova. Takođe, utvrđeno je postojanje
korelacija FIND i Prizma 7 skorova sa skorovima Upitnika za procenu nutritivnog
rizika („DETERMINE“), kognitivne procene (MoCA) kvaliteta sna (PSQI) i simptoma
depresije (GDS).
Zaključak: Postoji povezanost kognitivnog, emocionalnog i fizičkog
funkcionisanja kao i navika u ishrani kod osoba starijeg životnog doba. U skladu sa
tim potvrđujemo da određene fizičke – motoričke performanse kao što su brzina
hoda, dinamometrijsko ispitivanje snage stiska šake, mogu biti korisna sredstva u
gerijatrijskoj praksi u praćenju ne samo fizičkog, već i stanja kognitivne funkcije i
njihovog opadanja. S obzirom na povezanost kognitivnih, emocionalnih, fizičko
motoričkih karakteristika, kao i funkcije spavanja, staračku nemoć odnosno
fragilnost, možemo definisati kao stanje koje povezuje sve navedene karakteristike.
Rezultati ove studije, na indirektan način ukazuju na potrebu da u koncept
definisanja fragilnosti budu uključene kategorije koje se odnose na kognitivno i
emocionalno funkcionisanje, način ishrane i funkciju spavanja.
Introduction: Aging is a universal phenomenon and as such, arouses general social interest.
From the aspect of neuroscience, aging is a subject of interest, due to neurodegenerative
changes that can lead to changes in cognitive, psychological and physical functioning.
Understanding the cause-and-effect relationships between deteriorating mental and physical
processes associated with aging, which can potentially lead to loss of reserve capacity or
fragility, is critical, as it can help improve preventive and therapeutic strategies. The main
goal of this dissertation was to investigate the interrelation of cognitive, emotional, physical
functioning and nutritional habits in the elderly as well as the connection of these categories
with physical activity and thus, to more closely determine the characteristics of senile frailty.
Methods: Correlation, transversal study that included 140 subjects aged 65 and over, of both
sexes. Out of the total number of subject, two group...s were formed according to the criterion
of engaging in physical activity. Body mass index (BMI) was calculated after measuring
height and weight of subjects. The following tests and scales for assessment of cognitive
functions were used: Montreal Cognition Scale (MoCA), Go-go test, Categorical and
phonemic fluency tests. Physical-motor functioning was assessed using: Dynamometric
methods of hand grip, Gait Speed test, Berg balance scale (BBS), Timed up and go test
(TUG), Senior fitness test (SFT Questionnaires were also used in the research: Questionnaire
for nutritional screening "DETERMINE" as well as questionnaires for identifying fragility
and disability "FIND" and Prisma 7.
Results: Physically active subjects have on average a slightly lower BMI, and this difference
is statistically significant. The intercorrelations of the cognitive dimensions of the MoCA test
(executive functions, language, visuospatial skills, memory, attention, concentration, working
memory, and orientation) are positive, moderate to high, and most correlations are statistically
significant. Thus, higher scores on one dimension mean higher scores on the remaining
dimensions. A subgroup of persons engaged in recreational physical activity shows
statistically significantly better results on the MoCA, on the "Go-not-go" test, as well as on
the Categorical and Phonemic Fluency Tests. Subjects who are physically active achieve
significantly lower scores than subjects who are not physically active on the GDS, ie,
symptoms of depression are more pronounced in physically inactive subjects., The group of
physically active has statistically significantly lower scores on the PSQI, than the group of
physically inactive, which means that overall it has better sleep quality. In terms of physicalmotor tests, physically active subjects have better performance (perform more assigned
actions for a certain time, ie they need less time for a given action) on all motor tests, and the
difference in relation to the achievement of physically inactive is statistically significant
according to parametric and nonparametric analysis. The dynamometric parameters of hand
grip, indicate that the maximum hand grip force (Fmax) of both hands was related to walking
speed - subjects who took less time to cover a given distance demonstrated greater maximum
muscle force on both hands. Certain cognitive categories, are associated with greater hand
grip strength. There are correlations between cognitive functions and symptoms of depression,
and accordingly, the score on the MoCA is most strongly connected with depression. Subjects
who have better motor status (shorter time, more repetitions, etc.) also have lower scores on
the sleep quality questionnaire, which indicate better sleep. Subjects who have higher scores
on the scale of depression have a slightly worse motor status. Correlations were found
between FIND and Prisma 7 scores and: maximum hand grip of the left and right hand; TUG,
walking speeds, SFT and BBS scores. Also, the existence of correlations between FIND and
Prisma 7 scores and scores of the Nutrition Risk Assessment Questionnaire
("DETERMINE"), Cognitive Assessment (MoCA) of Sleep Quality (PSQI) and Depression
Symptoms (GDS) was found.
Conclusion: There is a connection between cognitive, emotional and physical functioning as
well as eating habits in the elderly. Accordingly, we confirm that certain physical - motor
performances, such as walking speed, dynamometric examination of hand grip strength, can
be useful tools in geriatric practice in monitoring not only physical, but also the state of
cognitive function and their decline. Given the connection between cognitive, emotional,
physical and motor characteristics, as well as sleep functions, we can define senile frailty, as a
condition that connects all these characteristics. The results of this study indirectly indicate
the need to include in the concept of defining frailty, categories related to cognitive and emotional functioning, nutrition and sleep function.
