Pharmacokinetics of 25-hydroxy vitamin D in children with asthma

dc.description.abstractUvod: Astma i deficijencija vitamina D su rasprostranjeni kod pedijatrijske populacije i adolescenata. Cilj ovog istraživanja je razvijanje populacionog farmakokinetičkog modela i utrđivanje faktora koji imaju signifikantan uticaj na klirens 25-hidroksi vitamina D kod dece sa astmom korišćenjem analize populacione farmakokinetike (PFK). Takođe, jedan od ciljeva je i ispitivanje potencijalnih faktora koji utiču na opštu anksioznost, njene specifične oblike, depresiju kao i na kvalitet života kod navedene populacije. Ispitanici i Metode istraživanja: Studijsku populaciju su činila deca (n=74) u školskom uzrastu i adolescenti od 7 do 18 godina koja boluju od astme. PFK analiza je koristila softverski program NONMEM (Non linear mixed effect model) pri čemu je ispitivano 19 kovarijanti. Procena anksioznosti, depresije i kvaliteta života je procenjivana korišćenjem specijalnih upitnika. Statistička analiza je izvedena korišćenjem SPSS softvera, kao i pomoću deskriptivne statistike i logističke regresije. Rezultati: Konačni model PFK analize je pokazao da faktori koji utiču na klirens vitamina D kod ispitavne populacije su unos vitamina D preko hrane, nasledni faktor i odnos forsiranog ekspirijumskog volumena u prvoj sekundi i forsiranog vitalnog kapaciteta (FEV1/FVC). Približno 33.8% ispitanika je pokazalo pozitivne rezultate na anksiozni poremećaj, od čega je najzastupljenija separaciona anksioznost (49.3%). Kod samo tri ispitanika su zabeležen depresivni poremećaj pri čemu se nijedna kovarijanta nije pokazala kao značajna. Dva faktora (GINA klasifikacija i indeks telesne mase) pokazala su statistički značajan uticaj na ukupni kvalitet života ispitanika. Nije utvrđeno postojanje korelacije između serumskih koncentracija 25-hidroksi vitamina D i anksioznosti, depresivnosti i procenjenog kvaliteta života kod dece obolele od astme, kao ni postojanje povezanosti između serumskih koncentracija 25-hidroksi vitamina D i postignutog uspeha u školi. Zaključak. Dobijeni rezultati mogu pomoći individualnom doziranju suplemenata vitamina D kod ove vulnerabilne populacije. Takođe, različiti faktori mogu biti povezani sa anksioznošću, kao i sa kvalitetom života ispitanika, pri čemu se na neke od njih može i uticati.sr
dc.description.abstractIntroduction: Asthma and vitamin D deficiency have been expanded in pediatric populations and adolescents. The aim of this study is development of a population pharmacokinetic model and determining factors that have a significant effect on 25-hydroxy-vitamin D clearance in children with asthma using population pharmacokinetics (PPK) analysis. Also, one of the goals is to examine potential factors that indicate general anxiety and its forms, depression, and the quality of life among children with asthma. Patients and Methods: The study population consisted of children (n = 74) in school age and adolescents aged 7 to 18 who are suffering from asthma. The PPK analysis was performed by the NONMEM software (Non linear mixed effect model), with 19 covariances tested. Assessment of anxiety, depression and quality of life was assessed using special questionnaires. Statistical analysis was performed using SPSS software, as well as descriptive statistics and logistic regression. Results: The final PPK model has shown that factors affecting vitamin D clearance in the test population are vitamin D intake via food, hereditary factor and ratio of forced expyrian volume in the first second and forced vital capacity (FEV1 / FVC). Approximately 33.8% of respondents showed positive results on anxiety disorder, of which the most common is separation anxiety (49.3%). Only three respondents reported a depressive disorder, with none of the covariates being significant. Two factors (GINA classification and body mass index) showed a statistically significant impact on the overall quality of life of the respondents. There was no correlation between serum concentrations of 25-hydroxy vitamin D and anxiety, depression, and estimated quality of life in children with asthma, or the existence of correlation between the serum concentrations of 25-hydroxy vitamin D and the achieved success in school. Conclusion. The results obtained can help individual dosage of vitamin D supplements in this vulnerable population. Also, different factors may be associated with anxiety, as well as with the quality of life of the respondents, with some of them being preventable.en
