dc.description.abstractHronična opstruktivna bolest pluća (HOBP) je sporo progresivno oboljenje koje se karakteriše ograničenim protokom vazduha kroz disajne puteve, koje nije potpuno reverzibilno. Ovo ograničenje je obično udruženo sa oksidativnim stresom i hroničnim inflamatornim odgovorom pluća na različite štetne nokse, među kojima duvanski dim ima najznačajniju ulogu. Merenje biomarkera oksidativnog stresa, 8-okso-7,8-dihidro- 2'-deoksiguanozina (8-oxodG) u urinu, kao produkta oksidativnog oštećenja DNK, značajno je za procenu sistemskih efekata oksidativnog stresa. Za procenu red-ox statusa organizma koriste se dva pristupa: prvi obuhvata merenje ukupnog oksidativnog statusa (TOS) i ukupnog antioksidativnog kapaciteta (TAC), dok drugi podrazumeva izračunavanje oksidativnog indeksa (OSI). Stepen izloženosti duvanskom dimu procenjuje se kvalitativno (na osnovu podataka o pušačkom statusu), kvantitativno određivanjem komponenti duvanskog dima ili njegovih metabolita (određivanjem urinarnog kotinina) i određivanjem kumulativnog dejstva duvanskog dima (izračunavanje indeksa paklo/godine). Razlike u preosetljivosti za nastanak HOBP-a, uslovljene su genskim varijantama enzima uključenih u detoksikaciju produkata duvanskog dima. U prvoj fazi metabolizma, pod dejstvom CYP P-450 enzima, štetne supstance se prevode u reaktivne intermedijere, dok se u drugoj fazi, pod dejstvom mikrozomalne hidrolaze epoksida (mEPHX) i glutation-S-transferaze (GSTs), vrši njiihova detoksikacija. Cilj ove studije bio je ispitivanje genetskih i ne-genetskih faktora rizika, povezanih sa povišenim oksidativnim stresom, kao i njihovih interakcija, u nastanku i razvoju hronične opstruktivne bolesti pluća (HOBP) u populaciji Srbije. Takođe, cilj je bio da se ispita i potencijalni značaj biomarkera koji su povezani sa oksidativnim stresom u dijagnostici i predikciji težine HOBP-a. U istraživanju je učestvovalo 195 pacijenata obolelih od hronične opstruktivne bolesti pluća (89 žena i 106 muškaraca) i 326 zdravih ispitanika koji su činili kontrolnu grupu (134 žene i 192 muškarca)...sr
dc.description.abstractChronic obstructive pulmonary disease (COPD) is a slow progressive disease characterized by limited airflow through the airway, which is not completely reversible. This limitation is usually associated with oxidative stress and chronic inflammatory response to various harmful agents, among which tobacco smoke plays the most important role. Measurement of biomarker of oxidative stress, 8-oxo-7,8-dihydro-2'- deoxyguanosine (8-oxodG) in urine, as a product of oxidative DNA damage, is important for assessing the systemic effects of oxidative stress. Two approaches are used to evaluate the red-ox status of the organism: the first involves the measurement of total oxidative status (TOS) and total antioxidative capacity (TAC), while the second involves the calculation of the oxidative index (OSI). The degree of exposure to tobacco smoke is assessed qualitatively (based on smoking status data), quantitatively by determining tobacco smoke components or its metabolites (by determining urinary cotinine) and determining the cumulative effect of tobacco smoke (calculating the pack/years index). Differences in hypersensitivity to the occurrence of COPD are conditioned by gene variants of enzymes involved in detoxification of tobacco smoke products. In the first phase of metabolism, under the action of CYP P-450 enzymes, the harmful substances are translated into reactive intermediates, while in the second phase, under the action of microsomal hydrolases of epoxide (mEPH) and glutathin-Stransferase (GSTs), their detoxification is carried out. The aim of this study was to investigate genetic and non-genetic risk factors associated with elevated oxidative stress, as well as their interactions, in the occurrence and development of chronic opstructive pulmonary disease (COPD) in Serbian population. Also, the aim was to examine the potential significance of biomarkers associated with oxidative stress in the diagnosis and prediction of COPD severity.
