Uvod/Cilj. Cilj ove studije je da utvrdi da li se koncept dnevne hirurgije štitaste žlezde
može bezbedno primeniti na gerijatrijsku populaciju pacijenata.
Metode. U ovoj retrospektivnoj kohortnoj studiji biće analizirani podaci svih
pacijenata kod kojih je učinjena hemitiroidektomija ili totalna tiroidektomija, u
periodu od januara 2012. do decembra 2018. godine, dok su iz studije isključeni pacijenti
kod kojih je istovremeno urađena tiroidektomija i disekcija limfnih čvorova vrata,
reoperacije štitaste žlezde, mlađi od 20 godina kao i pacijenti sa procenjenim ASA
skorom višim od 3. Eksperimentalnu grupu (gerijatrijska populacija) čine pacijenti
stariji od 65 godina i ona je uparena sa dve starosne kontrolne grupe (20-44 godina i 45-
64 godina) u odnosu 1: 1: 1, tako da broj ispitanika u eksperimentaloj i u kontrolnim
grupama iznosi po 247.
Rezultati. Broj pacijenata uključenih u ovu studiju je 741. Kod 642 pacijenta učinjena je
totalna tiroidektomija, a kod 99 pacijenata je učinjena h...emitiroidektomija. U
gerijatrijskoj populaciji češće je dolazilo do pojave otoka u predelu vrata i povećane
drenaže u odnosu na kontrolnu grupu. Svakom kasnijom godinom intervencije operisani
pacijenti imaju za 22% manju šansu za nastanak komplikacija, a benigne bolesti štitaste
žlezde imaju manje šanse za nastanak komplikacija.
Zaključak. Dnevna hirurgija sa produženim oporavkom je primenljiva u svim starosnih
grupama kod pacijenata sa hemitiroidektomijom kao i kod pacijenata mlađih od 65 godina
sa totalnom tiroidektomijom, dok se kod starijih pacijenata sa totalnom tiroidektomijom
dani hospitalizacije mogu produžiti zbog češćih hirurških i nehirurških
komplikacija.
Background/Aim. The objective of this study was to assess the safety of thyroid surgery for elderly
patients within a short-stay surgical setting.
Methods.
In this retrospective cohort study, we examined the medical records of all patients who underwent
total or hemithyroidectomy between January 2012 and December 2018, while all patients who
have undergone thyroidectomy and neck lymph node dissection at the same time, reoperations of
the thyroid gland, patients under 20 years of age, as well as patients with an estimated ASA score
higher than 3, were excluded from the study. The experimental group (geriatric population)
consists of patients older than 65 years and it is paired with two age control groups (20-44 years
and 45-64 years) in a 1:1:1 ratio, so that the number of subjects in the experimental and control
groups amounts to 247 each.
Results. The number of patients included in this study is 741. In 642 patients, a total thyroidectomy
was performed, and 99 patients had a hemithyr...oidectomy. Among the geriatric population, there
was a higher incidence of neck swelling and increased drainage output compared to the control
group, resulting in a greater need for extended hospital stays. The study also revealed that with
each subsequent year of intervention, the likelihood of developing complications decreased by
22%. Additionally, it was found that benign thyroid disease had a lower likelihood of leading to
complications.
Conclusions. Total thyroidectomy can be safely conducted as a short-stay procedure in patients
under the age of 65. However, in elderly individuals, the duration of hospitalization may be
prolonged due to a higher occurrence of both surgical and non-surgical complications. On the other
hand, hemithyroidectomy is considered safe across all age groups.
