Prosečan životni vek je uvećan poslednjih godina, i nastaviće da se uvećava. Sa
starenjem dolazi do pojave degenerativnih oboljenja, kao što je artritis, koji može
izazvati hronični bol u kuku i otežati kretanje. U odmaklim fazama oboljenja, jedini
način da se poboljša kvalitet života je da se ugradi veštački kuk. Totalna
artroplastika kuka je veoma uspešna operacija, koja može da ublaži bol i vrati
pokretljivost. Prilikom totalne artroplastike kuka, neophodno je izabrati između
cementnih i bescementnih implanta. Životni vek bescementnih implanta zavisi od
jačine veze koja je formirana između kosti i implanta. Postoji mnogo faktora koji mogu
uticati na kvalitet te veze, a jedan od njih je topografija površine implanta, što je i
bila glavna tema ove disertacije. U ovoj tezi, primenjena je metoda konačnih elemenata,
u cilju analize uticaja različitih topografija na distribuciju smičućeg napona
implanta. Veliki napon smicanja negativno utiče na okolno tkivo, tako da je cilj
koristiti topo...grafije, kod kojih je napon smicanja što manji. Primena ove metode nam
omogućava uštedu vremena i jednostavnije preliminarno testiranje različitih
topografija. Ovo pomaže izboru najbolje topografije na površini implanta, koja se
nakon toga analizira u laboratoriji. Nakon numeričkih simulacija, fokus disertacije
je bio na eksperimentalnom radu, koji se sastojao iz dva dela – kreiranje modifikovane
površine implanta primenom metode elektronskog snopa i in vitro evaluacija
vijabilnosti ćelija na modifikovanoj površini. Na kraju disertacije, prikazana je i
primena algoritama veštačke inteligencije, u cilju procene različitih topografija,
pre njihove numeričke i eksperimentalne analize.
Average life expectancy has increased in recent years and will continue to increase. Aging
brings the onset of degenerative diseases, such as arthritis, which can cause chronic hip pain
and make movement difficult. In the advanced stages of the disease, the only way to improve
the quality of patient’s life is to implant an artificial hip. Total hip arthroplasty is a very
successful procedure, which can relieve pain and restore mobility. During total hip arthroplasty,
it is necessary to choose between cemented and cementless implants. The lifespan of cementless
implants depends on the strength of the bond formed between the bone and the implant. There
are many factors that can affect the quality of that connection, and one of them is the topography
of the implant surface, which was the main topic of this dissertation. In this dissertation, the
finite element method is applied to analyze the influence of different topographies on the
distribution of the implant's shear stress. The goal i...s to use topographies where the shear stress
is as low as possible, as high shear stress negatively affects the surrounding tissue. Application
of this method allows us to save time and simplify preliminary testing of different topographies.
This helps to select the best topography on the implant surface, which is then analyzed in the
laboratory. After the numerical simulations, the focus of the dissertation was on the
experimental work, which consisted of two parts - creation of a modified implant surface using
the electron beam method and in vitro evaluation of cell viability on the modified surface. At
the end of the dissertation, the application of artificial intelligence algorithms is shown, in order
to evaluate different topographies, before their numerical and experimental analysis.
