dc.description.abstractKarcinom dojke najčešće metastazira u kosti. Vizuelizacione metode se koriste za dijagnozu metastaza i procenu rizika za patološki prelom. Biopsijom i patološkom analizom utvrđuje se status ER, PR i HER2 koji su ključni za prognozu i izbor terapije. Ciljevi studije bili su utvrđivanje kliničko-radiografskih karakteristika i imunohistohemijskog statusa ER, PR i HER2 i proliferativnog indeksa Ki67 skeletnih metastaza karcinoma dojke, i ispitivanje njihove povezanosti sa nastankom patološkog preloma. U studiju su uključene 152 pacijentkinje. Iz istorija bolesti dobijeni su klinički podaci i analizirane vizuelizacione dijagnostičke metode. Sve pacijentkinje su operisane i uzorci tkiva tumora su dalje analizirani za ER i PR prema Alred skoru. Pozitivnost HER2 utvrđena je na osnovu intenziteta bojenja i procenta obojenih ćelija. Indeks proliferacije Ki67 predstavljen je kao procenat obojenih ćelija tumora. Starost pacijenta, vreme proteklo od primarne bolesti do metastaza i vreme od dijagnoze skeletnih metastaza do skeletnog događaja nisu pakazali statistički značajnu povezanost sa nastankom patološkog preloma. Za broj i vrstu metastaza (mešovite, litične) takođe nije utvrđena statistički značajna povezanost sa prelomom. Najčešće slomljena kost bio je femur, praćen pršljenovima i humerusom. Statistički značajna povezanost sa nastankom patološkog preloma utvrđena je za duge kosti, posebno vrat i dijafizu femura i dijafizu humerusa. Statistički značajna povezanost sa prelomom utvrđena je za PR+ metastaze. Povezanost nije utvrđena za ER i HER2, kao ni za vrednost Ki67 indeksa. Ove rezultate mogu koristi ortopedski hirurzi za izbor pacijenata za profilaktičku operaciju i za to se posebno izdvajaju pacijenti sa PR+ metastazama u femuru i humerusu.sr
dc.description.abstractBreast cancer most commonly metastasizes to bones. Visualization methods are used to diagnose metastases and assess risk of pathological fracture. Biopsy and pathological analysis determine status of ER, PR and HER2 which are crucial for prognosis and treatment selection. Objectives of this study were to determine clinical-radiographic characteristics and immunohistochemical status of ER, PR, HER2 and the proliferative index Ki67 in skeletal metastases of breast cancer and investigate their association with occurrence of pathological fracture. Study included 152 patients. Clinical data were obtained and visualization diagnostic methods were analyzed from medical records. All patients underwent surgery and tumor tissue samples were analyzed for ER and PR according to the Allred score. HER2 positivity was determined based on the intensity of staining and the percentage of stained cells. The Ki67 proliferation index was presented as percentage of stained tumor cells. Patient age, time from primary disease to metastasis and time from diagnosis of skeletal metastases to skeletal event did not show statistically significant association with occurrence of fractures. Number and type of metastases (mixed, lytic) also did not show statistically significant association with fractures. The most commonly fractured bone was femur, followed by vertebrae and humerus. Statistically significant association with occurrence of fracture was found for long bones, particularly neck and diaphysis of femur and diaphysis of humerus. Statistically significant association with fractures was found for PR+ metastases. No association was found for ER and HER2, nor for Ki67 index. These results can be used for prophylactic surgery, particularly for patients with PR+ metastases in femur and humerus.en
