Uvod: Cerebrovaskularne bolesti (CVB) najčešće su udružene sa brojnim
komorbiditetima, čija se težina i rizik od intrahospitalne smrti mogu procenjivati
Čarlsonovim (CCI) i Elikshauserovim (ECI) indeksom komorbiditeta. Nakon uvođenja
sistema dijagnostički srodnoh grupa (DSG), a pre započinjanja plaćanja bolničkih
usluga po DSG-u, poželjno je analizirati varijacije u kodiranju komorbiditeta
hospitalizovanih pacijenata. Cilj studije je analiza performansi komorbiditeta u
sistemu dijagnostički srodnih grupa kod hospitalizacija uzrokovanih
cerebrovaskularnim bolestima.
Metod: Retrospektivno-prospektivna studija tipa kohorte. Uzorački okvir su Baza
hospitalizacije Kliničkog centra Kragujevac i Baza umrlih u Šumadijskom upravnom
okrugu u periodu 2014-2020. godina. Iz baze hospitalizacije izdvojeni su punoletni
pacijenti hospitalizovani usled CVB, završna dijagnoza. Iz baze umrlih izdvojeni su
svi preminuli usled CVB, osnovnim uzrokom smrti. Analiza prediktivne moći CCI i
ECI testirana je C-s...tatistic-om, a upoređena na osnovu površine ispod ROS krive. Cox
proportional hazards regression upotrebljena je kako bi se identifikovali riziko
faktori smrtnog ishoda kod otpuštenih pacijenata.
Rezultati: Nakon početnog stagniranja uočava se trend porasta registrovanja
komorbiditeta, od momenta uvođenja DSG, uz porast CCI i ECI skorova. Prisustvo
komorbiditeta i viši CCI i ECI skorovi povezani su sa dužim boravkom pacijenata u
bolnici i letalnim ishodom hospitalizacije. ECI model ima bolju prediktivnu moć
letalnog ishoda hospitalizacija. Kod otpuštenih pacijenata svaka godina života
povećava verovatnoću letalnog ishoda za 7%, a povećanje ECI skora za 3%.
Zaključak: Nakon početnog stagniranja, beleženje komorbiditeta i vrijednosti CCI i
ECI povećavaju se iz godine u godinu. Veći CCI i ECI skorovi povezani su sa dužom
hospitalizacijom i većom stopom mortaliteta. Veću prediktivnu moć ima ECI skor.
Prediktori smrti kod otpuštenih CVB pacijenata su starost i vrednost ECI.
Introduction: Cerebrovascular diseases (CVD) are usually associated with numerous
comorbidities, whose severity and risk of intra-hospital death can be assessed by the Charleson
(CCI) and Elixhauser (ECI) comorbidity index. After introducing the system of the Diagnosis
Related Groups (DRG), and before starting to pay for hospital services according to the DRG,
it is desirable to analyze variations in the coding of comorbidities of hospitalized patients. The
aim of the study is to analyze the performance of comorbidities in the system of diagnostically
related groups in hospitalizations caused by cerebrovascular diseases.
Method: A retrospective-prospective cohort-type study. The sampling frame consisted of the
Hospitalization Base of the Clinical Center of Kragujevac and the Database of Deaths in the
Šumadija Administrative District in the period 2014-2020. years. We singled out adult patients
from the hospitalization base who had been hospitalized due to CVD- the final diagnosis. All
...deaths due to CVD, as the main cause of death, were separated from the database. The
predictive power analysis of CCI and ECI was tested by C-statistic, and compared based on the
area under the ROS curve. Cox proportional hazards regression was used in order to identify
risk factors for death in the discharged patients.
Results: After the initial stagnation, there is a trend of increasing comorbidity registration, after
the introduction of DRG, with an increase in CCI and ECI scores. The presence of
comorbidities and higher CCI and ECI scores were associated with longer hospital stays and
lethal outcome of the hospitalization. The ECI model has better predictive power of the lethal
outcome of hospitalizations. In discharged patients, each year of life increases the probability
of death by 7%, and the increase in ECI score by 3%.
Conclusion: After the initial stagnation, the recording of comorbidities and CCI and ECI
values increases from year to year. Higher CCI and ECI scores are associated with longer
hospitalizations and higher mortality rates. The ECI score has higher predictive power.
Predictors of death in discharged CVD patients are age and ECI value.
